Items in the Game
Here you will find every item in the game — what it does, why it matters, and how it differs between versions of the game.
Scoring
Picking up certain items affects both your running score during the level and the bonus points you receive at the end of it. End-of-level bonuses are awarded as follows:
- Kill every enemy on the level — 10,000 bonus points
- Collect every treasure on the level — 10,000 bonus points
- Find every secret passage (pushwall) — 10,000 bonus points
- Extra life — awarded every 40,000 points scored (maximum of 9 lives)
Keys

Keys unlock doors that block your progress through the level. They can be hidden almost anywhere — sometimes tucked away in a secret room, sometimes sitting in plain sight, and occasionally carried by a boss enemy. If you come across a locked door, the key for it is always somewhere on the same floor. Keep exploring until you find it.
Food / Chicken Meal

A basic meal that restores a small amount of health. You can only pick it up if your health is below 100%, so there is no point grabbing it when you are already at full health. If you spot a food item but don't need it yet, try to remember where it was — it could save your life later in the level.

Dog Food

Exactly what it sounds like. Not exactly appetizing, but it restores a small amount of health when you need it. Like the food plate, you can only pick it up when your health is below 100%. Don't be too proud to eat it — in a desperate situation, every little bit counts.
- Health restored: +4%
First Aid Kit

The most effective health item in the game. A first aid kit restores a significant amount of health and can turn the tide in a tough fight. As with all health items, you can only pick it up if your health is below 100%. Save these for when you really need them.
- Health restored: +25%
Blood / Gibs / Bloody Skeleton

These are pools of blood, scattered remains, or bloody skeletons left behind after combat. They can only be used when your health has dropped to 10% or below — think of them as a last resort. They restore very little health, but in a desperate situation they may be just enough to keep you alive until you find something better.
- Health restored: +1% (only usable when health is at 10% or below)
Extra Life / Power Up (Full Heal)

One of the most valuable items in the game. These fully restore your health, give you extra ammunition, and add an extra life. They are almost always hidden behind pushwalls and can be very difficult to find. If you discover one during a boss fight or right after one, consider yourself very lucky. Note that these items count as treasures for the purpose of your end-of-level bonus.
- Health restored: +100%
- Ammunition: +25 rounds
- Lives: +1
Machine Gun

A significant upgrade over the starting pistol. The machine gun is more accurate and far more effective at longer ranges. You can sometimes find one hidden in a secret area or behind a pushwall, but the quickest way to get one is to kill an SS guard and pick up the weapon he drops. If you don't have a machine gun yet, getting one should be a priority.
- Ammunition when picked up: +6 rounds
Chaingun

The most powerful weapon in the game. The chaingun fires extremely fast and is essential when facing a boss or a large group of SS guards. However, it burns through ammunition very quickly, so avoid using it on weaker enemies — it is overkill and a waste of bullets. Chainguns are only found hidden in secret areas or behind pushwalls, so they take effort to find. Once you have one, hold onto it.

Ammunition Clip

Ammunition clips can be found scattered around levels or dropped by guards you have killed. You can only pick them up when you have fewer than 99 rounds. When you are fully stocked, make a note of where you saw them — you will want to come back for them later.
- Ammunition (picked up from the floor): +8 rounds
- Ammunition (taken from a killed guard): +4 rounds
Ammunition Chest (Spear of Destiny only)

Ammunition chests were introduced in Spear of Destiny and its mission packs. They provide a much larger amount of ammunition than a standard clip and are typically placed near boss encounters or in other locations where you are likely to need extra firepower. If your ammunition is already at maximum when you find one, remember where it is and come back when you need it.

Treasures

Treasures don't help you survive, but they dramatically boost your score. Each treasure is worth a set number of points when collected, and finding every treasure on a level earns you an additional 10,000 bonus points at the end. It is always worth going out of your way to collect them.
The names and appearances of treasures differ between Wolfenstein 3D and Spear of Destiny, but their point values are the same:
